Flip Flop...Flip Not! | NJ Podiatrist

It is very tempting to take advantage of that great sale on flip flops and purchase a pair in every color. When you’re at the mall and all those spring and summer outfits are calling your name, what could be easier than going into the store that has that big 10 for 10 deal and getting a bunch of flip flops? That might sound like the easiest and cheapest idea to coordinate all of your new ensembles. That very same decision could also be the cause or your future foot problems and cost you way more than a good pair of sandals.
Flip Flops provide absolutely NO support for the foot. There is no arch support, no less any kind of arch. There is no control of the heel being that the only thing keeping it on your foot in a tiny little thong between your big toe and your second toe. Don’t forget about what happens if your flip flop gets a little wet and you have one little misstep. BOOM! You fall, or twist that wonderful little thong between your toes. Ankle sprain or strains are also a common when you try too many crazy moves in a flip flop. Bunions can also get worse if you already have them or start to form when wearing flip flops all the time. With no heel control or arch support, your gait is affected and could cause your toes to migrate in directions that are not only painful but very unattractive.
Here in the office we offer great looking sandals that are also great for your feet! Don’t worry that they are unattractive too…they happen to be adorable and come in a wide range of styles and colors! The “Aetrex Sandalista” collection is designed with an anatomically shaped and contoured footbed to match the foot and eliminate friction. They also feature an accommodative orthotic known as a “Lynco” which is customizable for personalized comfort. We also offer the Sandalista collection with a cork sole which incorporates memory foam that conforms to your specific gait pattern and ensures an equal distribution of pressure while you walk.

To learn more about how your shoes may be runing your feet and causing unnecessary pain, click here!

Medicare Deductibles, Secondary Insurances and Who Covers What?

Every year Medicare patients have a deductible that must be satisfied. The deductible amount changes every year. Many patients have secondary insurances to supplement the balance of what Medicare does not cover.

This is where the confusion starts for many. You may ask yourself, why if a patient has a secondary insurance, do I still get bills from what Medicare doesn’t cover? The truth is every insurance plan is different. Some cover deductibles, some cover the full Medicare 20% co-insurance, some do not cover either.

When selecting your Medicare supplement plan make sure to ask all the questions first. Find out how much, if any of the Medicare deductible will be covered. Also, check if all or part of the Medicare 20% co-insurance is covered. You may have to pay more for the plans that cover your deductible and co-insurance, but if being free from any additional bills is important to you, then that would be your best option. Paying more on a monthly basis for a better supplemental plan will allow you to budget your finances accordingly, rather than getting a surprise bill for more than you planned.

Each patient should take time to understand their insurance. Weighing your options based on your health needs and financial situation is an important step to taking charge of your healthcare.

Ankle instability on the outside or lateral ankle is very common with high arched feet. Any sudden twist or turn can cause the ankle to turn in or INVERT. If the twist has enough energy it can lead to an acute lateral ankle sprain. If the force is very high, then this can lead to an ankle fracture, which can require surgical repair of the fracture.

2. Stress fractures, especially of a bone called the fifth metatarsal, is another condition associated with high arched feet. This is not caused by one specific injury rather it is due to repetitive use. If you are on your feet a lot, you started a new activity or job, you started walking or running on a hard surface or if you are constantly on your feet for work ,you can develop this condition. With stress fractures the hard outside bone called the cortex is intact though the inside of the bone is cracked. These injuries do not always show on X-ray and if they do they are very subtle and can take 10-14 days to show up on the X-ray films. Initial treatment is immobilization in a walking boot for 4-6 weeks.

3. Tendonitis, most commonly of the outside tendons called the personal tendons, is another common condition associated with high arches. Tendonitis is swelling and pain of the tendon. Initial treatment is temporary immobilization and physical therapy to strengthen the tendon.
Joint pain is another common condition where there is a lot of pain in the subtler joint . This is the joint that lets your foot go side to side. Cortisone or steroid injections can help with the acute pain.

Treatment:
Long-term treatment for all of these conditions is simple. Customized orthotics (inserts) to functionally stabilize the foot to the ankle are essential to correct the root cause of the conditions. Orthotics will functionally “straighten” out the high arches allowing for more pressure in the arch area and less on the outside lateral foot and ankle. A softer semi-rigid orthotic is crucial for a high arch to provide shock absorption. This will reduce the pressure off the outside of the foot to prevent the tendonitis, stress injuries, fractures and sprains most common with high arches. Orthotics should also be utilized as prevention before a problem arises.

So if you, your child, or someone you know has high arches especially if that person is very active, you should visit your local podiatrist for a comprehensive foot evaluation. You will be advised to get customized orthotics, a process done in the podiatrist’s office where an impression is made of your foot.

Guide to Health insurance

Health insurance is a hot button issue in America today, yet so many don’t fully understand how it works. I am going to review some commonly asked and frequently confused topics.

When going through your benefits paper work you may come upon terms such as Flexible Spending Account and Health Savings Account and be unsure exactly what these terms mean or the difference between them.

A Flexible Spending Account or FSA is an option provided by some employers that allows you to set money aside for medical expenses and reduces your amount of taxable income.

You would choose an amount of pre-tax dollars to be set aside from your paycheck each pay period; this may be a set dollar amount or a percentage of your paycheck. You then may use this money for health related expenses through out the year, such as co-pays, deductibles, prescriptions etc. Some employers offer a FSA card that acts as a credit card. Others may require you save your receipts and submit them to your benefits administrator or the third party administrator handling your account.

It is important to note that with an FSA you will have a year to utilize these funds. This may be a plan year or a calendar year. Some employers have grace periods of 30, 60 or 90 days after the end of the year of which you can utilize these funds. However, once the year has passed and your grace period is over, any funds remaining in the account will be forfeited. This is why it is important to select an amount of holdings that will meet your needs. Also, the IRS does not allow medical expenses paid with your FSA to be claimed as a tax deduction.

Health Savings Accounts (HSA) were enacted by the Medicare bill signed by President Bush in 2003. This allows individuals with approved high deductibles health plans (HDHP) to save for medical expenses on a tax free basis. There are minimum deductible requirements and maximum contributions limits. If you are eligible, you can use these savings to pay for your medical expenses or those of your spouse or dependents. You can find out more information about FSA’s and HSA’s on the IRS. The Department of the Treasury also has a direct link to information about HSAs on its web site.

By: Sarah Avilleira

http://www.ustreas.gov/offices/public-affairs/hsa/

http://www.irs.gov/pub/irs-pdf/p969.pdf

Bunions

A bunion is a bony bump on the joint at the base of the big toe. As the bump gets bigger, it causes the big toe to turn in toward the second toe. The tissues around the joint may be swollen and tender.
You may get bunions if:
 The way your foot is shaped puts too much pressure on your big toe joint. Because bunions can run in families, some experts believe that the inherited shape of the foot makes some people more likely to get them.
 Your foot rolls inward too much when you walk.
 You have flat feet.
 You often wear shoes that are too tight.
All of these may put pressure on the big toe joint. Over time, the constant pressure forces the big toe out of alignment, bending it toward the other toes.

To help our patients get relief from bunion pain we would most likely recommend, wearing wider shoes, orthotics, foot pads, oral anti-Inflammatories, and injections.
Flat Feet

Flat Feet
Some people have flat feet (no arches) and it doesn't cause them any problems. However, more often than not, if you have flat feet you are very aware of it because it does create pain in your feet, legs and back. The medical term for flat feet is pes planus.
The aim of the arch in the foot is to give us spring and distribute our body weight across our feet and legs. The structures of the arches of our feet determine how we walk - they are rigid levels which allow us to move smoothly. However, the arches need to be sturdy as well as flexible to adapt to various surfaces and stresses.

Definition:
If you have flat feet, this means that your arch has collapsed or never developed. This can be an inherited condition, meaning you may be born this way or you may develop this condition when you get older. All babies are, in fact, born with flat feet because their arch hasn't yet developed. Sometimes it never develops. Flat feet can also develop due to an injury, wear and tear due to age or a medical condition (such as rheumatoid arthritis, cerebral palsy).
Flatfeet can cause problems and pain in your ankles, knees and back because the condition can force your ankles to turn inward and this throws off the alignment of your legs and back. Even if you aren't experiencing any pain, treatment in most cases is still necessary to prevent problems in the future.

Symptoms:
Symptoms may vary and generally depend on the severity of the condition. If your feet are flat, you may experience swelling and pain on the inside of your ankle, foot pain, lower leg pain and back pain. Another indicator that you have flat feet will be uneven wearing of your shoes, plus you will be able to see the outright appearance of a flat foot, which looks significantly different from a foot with an arch.

Diagnosis:
There are two different kinds of flat feet. If you have a rigid flat foot, that means that the foot is flat regardless of whether you are sitting or standing. Those with a flexible flat foot will have a bit of an arch when they are sitting down, but that arch disappears when the person stands up and puts weight on his feet. This can lead to ankle and foot pain as well as leg, hip and knee pain. Individuals with rigid flat feet generally experience more problems and symptoms. Most qualified health care professionals can diagnose flat feet just by watching the patient stand, walk and examining his/her feet. In some cases the physician may order an X-ray, CT (computed tomography) scan, or MRI (magnetic resonance imaging) scan.

Treatment Options:
• Well fitted and supportive shoes that are extra wide. Most patients are not wearing the right size or type of shoes for their feet.
• Fitted insoles or orthotics (custom-designed for each foot) relieve pressure from the arch and reduce pain. A good custom orthotic is an extremely valuable and simple treatment that will correct most patients’ flat feet.
• Wearing a specially designed brace may help patients that have painful tendonitis. The brace stops the foot and ankle from moving and allows the inflammation to calm down.
• Rest, Ice and anti-inflammatory medications
• Physical Therapy can strengthen the muscles around the foot and ankle and also increase range of motion in the joints. Thus, helping patients reduce their pain, inflammation and walk correctly.
• In some cases surgical intervention is required. There are many surgical procedure that will correct flat feet permanently, you doctor will best be able to determine which procedure is best for your particular case.
So to summarize, flat feet can be an extremely painful and debilitating condition that severely limit a person’s lifestyle. You can be born with flat feet or it may develop as you get older. Flat feet are very easy to diagnose and there are some very simple yet effective treatments. Again, even if flat feet don’t cause pain, they should still be treated in order to prevent problems from occurring in the future.
